Information obligation - For the patient

In accordance with Article 13(1) and (2) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46/EC (General Data Protection Regulation), hereinafter referred to as "RODO", I inform you that:

The administrator of your personal data is: Heals Sp. z o.o. Sándora Petöfiego 8 / 10, 01-917 Warsaw, Poland, KRS 0000780941,NIP 5272889763, REGON 383063446.

Your personal data will be processed:

(a) for: providing health care services, documenting health conditions, verifying eligibility for health care services, and for communication in the use of health care services.

(b) on the basis of: article 9(2)(h) of the RODO, the provisions of the Act on Patients' Rights and the Patients' Ombudsman, the Act on Health Care Services and on the basis of consent.

Recipients of your personal data will be: only entities entitled to obtain personal data on the basis of the Act on Patient's Rights and Patient's Ombudsman and other legal provisions (e.g. the police, the NFZ or any other entity financing healthcare services) and entities with which the Administrator has signed an agreement on entrustment of personal data processing.

Your Personal Data will be stored for the period: indicated in the Act on Patients' Rights and the Patients' Ombudsman and for the periods referred to in special laws, and in cases where consent has been given, until the date of withdrawal of consent.

You have the right of access to the content of your data and the right to rectification, erasure, restriction of processing, the right to data portability, the right to object, the right to withdraw your consent at any time without affecting the lawfulness of the processing (if processing is based on consent) that was carried out on the basis of consent before its withdrawal, taking into account the provisions on the collection and processing of data in medical records referred to in the Act on Patients' Rights and the Spokesman for Patients' Rights and in the provisions of special laws.

You have the right to lodge a complaint: to the President of the Data Protection Authority if you consider that the processing of your personal data violates the provisions of the RODO.

Your personal data will not be transferred to a third country/international organisation.

The provision of personal data collected in the medical records is mandatory and is a statutory requirement, and the consequence of failing to provide personal data will be the inability to provide health care services and to document health conditions. Otherwise, the provision of personal data is not mandatory.
